<br />HUD's Guidelines for Preparing a Consolidated Plan Submission for Local Jurisdictions (Guidelines) consolidates the <br />information within 24 CFR Part 91 into three sections: <br /> <br />1) <br />2) <br />3) <br /> <br />Citizen Participation <br />Strategic Plan, and <br />Annual Action Plan <br /> <br />However, the format of the FY 2005-2009 Consolidated Plan shall be formatted into the five sections as stated in 24 <br />CFR Part 91. <br /> <br />The final Consolidated Plan wiil be grammatically correct, free of spelling errors and acceptable to the City of Santa <br />Ana. <br /> <br />C. <br /> <br />Deliverables <br /> <br />The deliverables should include, but not be limited to the foilowing: <br />Creation of Survey materials <br />Collection and analysis of survey results and inclusion into the Consolidated Plan <br />Presentation boards for public workshops <br />Citizen Participation Plan to be included in the Consolidated Plan. <br />Draft Consolidated Plan (5 per submittal) <br />Final Draft Consolidated Plan for Public Review (10 copies) <br />Final Consolidated Plan (unbound) 1 Original <br />Scan the Final Consolidated Plan and provide a computer disc with the Plan formatted in Microsoft Word <br />Final Document acceptable to HUD <br /> <br />SECTION II: PROPOSAL RESPONSE REQ~ <br /> <br />Proposals should be prepared to provide straightforward, concise description of capabilities to satisfy the requirements of the <br />RFP. Emphasis should be on completeness and clarity of content with sufficient detail to allow for accurate evaluation and <br />comparative analysis. Responses must include: <br /> <br />A. <br /> <br />Cover LetterNaliditv Of Proposal <br /> <br />1. Ail proposals must be accompanied by a cover letter, signed by corporate officers or other persons authorized to <br />bind the proposing entity. An unsigned proposal submission is considered unresponsive. <br /> <br />2. The Offeror shall state the length of time for which the proposal shail remain valid. This length of time shall not be <br />less than two (2) months. <br /> <br />B. <br /> <br />Proposal Cost Summary <br /> <br />The proposal must contain a maximum not-to-exceed fee, which will include all labor, supplies, travel, meeting <br />expenses. The City will withhold ten (10) percent retention for each billing. This retention will be released after the <br />Consolidated Plan and related certifications are approved by HUD. <br /> <br />C. <br /> <br />Proiect Work Plan/Schedule <br /> <br />Offerors shall prepare a work plan.
