All original plan sheets, the title sheet of the specifications, calcutations, and reports shall be signed and stamped by <br />the Consultant's licensed professional architect/engineer in responsibte-charge ofthe project. These signed <br />originals will then become the property of the City. Drawings shall be per standard City formatted sheet, as <br />provided by City. <br />10. Upon comptetion of construction, as-builts shall be submitted to Consultant. Consultant shall incorporate all <br />changes to the plans etectronically with all necessary revision notations. Once plans have been updated, a signed <br />set of as-built mylars shall be submitted to the City with an electronic copy of the final as-built drawings via CD or <br />e-mail. <br />II. Consultant shall tightly control the quality of the work performed by in-house staff and/or sub-consultants. <br />Incomplete (not meeting targeted % completion) or poor quality work will not be accepted. The consultant will <br />need to revise the documents within a revised schedule set by the City, which may require overtime. No additional <br />regular or overtime hours will be approved for the required revisions. <br />3. PROJECT BUDGET <br />The project budget for removal of existing netting and designing, furnishing and installing a complete netting <br />system as detailed is $225,000. <br />4. CONTENT OF PROPOSAL <br />In order to maintain uniformity with all proposals furnished by vendor/contractor requested the proposals be <br />limited to a maximum of 15 pages (excluding front and back covers, section dividers and resumes) and <br />includes the following: <br />A. A project understanding statement containing any suggestions or special concerns that the City should be <br />made aware of, and a project approach containing any additional scope of work tasks you feel are <br />necessary for the successful comptetion of the project. <br />B. A list of simitar projects that your firm completed within the tast 5 years. Project information should <br />include project description; agency or client name along with the person to contact and the telephone <br />number(s), year completed and project construction and design cost. <br />C. Proposal submittal and signature. Five (5) copies of your proposal shall be signed by a company official <br />with the power to bind the company in its proposal. All proposals must be completely responsive to the <br />RFP. <br />5, CITY'S SELECTION COMMITTEE <br />The City has established a Selection Committee to evaluate vendor/contractor proposals. The evaluation of <br />each proposal will be based on technical criteria and qualifications listed within the RFP, reference checks, and <br />other information that will be gathered independently. A final listing ofthe most qualified consultants will then <br />be established for possible interview. <br />6. ESTABLISHING OF FEES <br />A fee proposal is to be submitted separately in a sealed envelope plainly labeled "Fee Proposal". The fee <br />proposal will not be opened until the proposals have been evaluated by the proposal selection committee.
