<br />maintain and shall I.'!:quire its subcontractors, if any, to maintain insurance as <br />described below: <br /> <br />I. Workers' Compensation Insurance with statutory limits. <br /> <br />2. Commercial general liability insurance, or equivalent form, with a <br />combined single limit of not less than $1,000,000 per occurrence. If <br />such insurance contains a general aggregate limit, such limit shall apply <br />separately to each project the Consultant performs for the City. Such <br />insurance shall: a) Name the City of Santa Ana and its officers and <br />employees as additional insureds; and b) Be primary with respect to <br />insurance or self-insurance programs maintained by the City, and c) <br />Contain standard separation of insureds provisions. <br /> <br />3. Professional liability (errors and omissions) insurance, with a combined <br />single limit of not less than $1,000,000 per claim. <br /> <br />~~. <br /> <br />Consultant shall: a) Furnish properly executed certificates of insurance to the <br />Executive Director of Public Works (Director) prior to commencement of work <br />for this project, which certificates shall clearly evidence all coverages required <br />above and provide that such insurance shall not be materially changed or <br />terminated except on 30 days' prior written notice to the City (See Sample <br />in the Appendix), b) Attach a completed and signed copy of the City's <br />"Additional Insured Endorsement" form to the certificates of insurance <br />noted above (See Sample in the Appendix), c) Maintain such insurance from <br />the time work first commences until completion of work for this project, and d) <br />Replace such certificates and form for policies expiring prior to completion of <br />work for this project. <br /> <br />The Director may waive or reduce the requirements of this section if and to the <br />extent the Director determines them to be unreasonably burdensome to the <br />Consultant and not necessary for the protection of the City. <br /> <br />Samples of the certificate of insurance and "Additional Insured Endorsement>> <br />form are in the Appendix. <br /> <br />G. Fee Proposal <br /> <br />I. The fee proposal must be bound and sealed, separately from the <br />proposal, and submitted to the City with the proposal. Consultant <br />compensation shall be on a fIXed fee basis. The fee proposal must be <br />segregated by the tasks listed below along with the hours assigned for all <br />personnel: <br /> <br />Task I. Radar speed surveys (180 zones) <br /> <br />5 <br />
