0 0 11. INFORMATION REQUIRED FROM CONSULTANTS A. The response to this Request for Proposal should contain documentation of Consultant's and/or Consultant's firm's credentials and expertise in this field. Substantial consideration will be given to consultants with demonstrable and documented experience in similar work. B. Proposals shall be submitted in the format described below: Organization and Credentials • Provide a synopsis of the Consultant's qualifications, including staffing, organizational chart, professional awards, and sub -consultants, if appropriate, • Identify the Project Manager as well as other key personnel to be assigned to the project, their qualifications, education, representative experience, professional awards, and their capability to explore and resolve problems. • Identify any contemplated subconsultants to be used during the project, including the personnel to be assigned, their qualifications, education, and representative experience. Note: Subcontractors cannot be used without written approval of the City. State whether or not the subconsultant(s), if any, is a minority, women or disabled business enterprise. • Provide a synopsis of the Consultant's past experience in similar work, • Demonstrate financial capability, • Identify City where assigned staff will be based, • Demonstrate reliability and continuity, and • State whether or not the firm is a minority, women, or disabled veteran business enterprise (M/W/DVBE). Project Understanding • Provide a precise rendering of the Consultant's understanding of the City's needs for the Consultant Services listed in Attachment A. Work Plan • Provide a narrative rendition of the technical work plans for the Consultant Services listed in Attachment A. • Provide a schedule of work for each Consultant Service listed in Attachment A. -5-
