References • Consultant shall provide a list of at least two (2) references of previous, recent related projects, with the name of the agency, the name of a contact person, address and telephone number. 12. ADDITIONAL INFORMATION AND COMMENTS The contents under this heading are left to the discretion of the Consultant. Material shall be pertinent to the proposal, but not otherwise requested in the RFP. 13. RULES FOR PROPOSALS The signer of the proposals must declare in writing that the only person, persons, company or parties interested in the proposal as principals, are named therein; that the proposal is made without collusion with any other person, persons, company, or parties submitting a proposal; that it is in all respects fair and in good faith without collusion or fraud, and that the signer of the proposal has full authority to bind the principal proposer. 14. SUPPLIED TO OFFEROR A. The offeror shall furnish a fee proposal for the proposed service. The fee proposal shall include the fee schedule and completed Attachment A, Consultant Rate Comparison Form. B. The fee schedule shall include the following: 1. Hourly rate for each personnel category, 2. Any other direct charges, 3. Indirect cost or overhead, and 4. Cost of supplies and materials (itemized). C. The fee proposal shall be bound and sealed separately from the proposal, and submitted to the City with the proposal. D. The fee for each assigned project will be negotiated following selection of the Consultants, at the time the project is assigned. E. The cost for each contract shall not exceed $125,000 per year or $250,000 total. 15. CRITERIA FOR SELECTION The responses received will be reviewed by a selection committee of City staff members. All proposals received will be fully reviewed and rated by the selection committee. The committee will then average the scores of each evaluator. Based on average scores, the top group of Consultants will be selected, and their sealed fee
