References <br />• Consultant shall provide a list of at least two (2) references of previous, <br />recent related projects, with the name of the agency, the name of a contact <br />person, address and telephone number. <br />12. ADDITIONAL INFORMATION AND COMMENTS <br />The contents under this heading are left to the discretion of the Consultant. Material <br />shall be pertinent to the proposal, but not otherwise requested in the RFP. <br />13. RULES FOR PROPOSALS <br />The signer of the proposals must declare in writing that the only person, persons, <br />company or parties interested in the proposal as principals, are named therein; that <br />the proposal is made without collusion with any other person, persons, company, or <br />parties submitting a proposal; that it is in all respects fair and in good faith without <br />collusion or fraud, and that the signer of the proposal has full authority to bind the <br />principal proposer. <br />14. SUPPLIED TO OFFEROR <br />A. The offeror shall furnish a fee proposal for the proposed service. The fee <br />proposal shall include the fee schedule and completed Attachment A, Consultant <br />Rate Comparison Form. <br />B. The fee schedule shall include the following: <br />1. Hourly rate for each personnel category, <br />2. Any other direct charges, <br />3. Indirect cost or overhead, and <br />4. Cost of supplies and materials (itemized). <br />C. The fee proposal shall be bound and sealed separately from the proposal, and <br />submitted to the City with the proposal. <br />D. The fee for each assigned project will be negotiated following selection of the <br />Consultants, at the time the project is assigned. <br />E. The cost for each contract shall not exceed $125,000 per year or $250,000 total. <br />15. CRITERIA FOR SELECTION <br />The responses received will be reviewed by a selection committee of City staff <br />members. All proposals received will be fully reviewed and rated by the selection <br />committee. The committee will then average the scores of each evaluator. Based on <br />average scores, the top group of Consultants will be selected, and their sealed fee <br />
