Therefore, continued involvement and peer <br />mentoring will serve as intermediate outcome measures. <br /> <br />Long-term outcomes (i.e., high school graduation, trade-school or college <br />emollment/completion): Projected percent of youth who achieve one or combination of <br />objective = 75% (n=22.5; N= 30) <br /> <br />The attention to literacy and numeracy competency, along with 3.D-BPA is for the <br />purpose ensuring high school graduation and subsequent job placement and success, trade <br />school or college bound trajectory. Thus, high school diploma, job placement and <br />success, and trade school or college enrollment/completion will serve as long-term <br />outcome measures. <br /> <br />GOALS/OBJECTIVES AND PERFORMANCE LEVLES <br /> <br />Basic Program Approach! Academic Assistance: <br /> <br />Membership on the boxing team has been an incentiye to the youth to maintain 2.0 GPA <br />We plan to capitalize on the galvanizing properties ofthe boxing actiyity by increasing <br />the required GPA to 3.0. In order to accomplish this we will offer 1 hour tutoring prior to <br />boxing activity. We currently have access to classrooms at Garfield Middle School, <br />across the street from out boxing club, to use for our after-school clubs. Therefore, it <br />should not pose a problem to get access to a classroom for the purpose of tutoring our <br />boxing club members. Moreover, we have teachers and student teachers available and <br />willing to tutor for this purpose. We would also utilize this time to increase literacy and <br />numeracy scores on the T ABE. <br /> <br />Case Managementllntake Assessment: <br /> <br />Anthony Serrano, Sports Club Director, serves as our case manager. He will provide <br />strong collaborative and strong case management that will enabling "at-risk" to transition <br />to a positive future through the sport of boxing, mentoring, tutoring. He will motivate and <br />coordinate services and information. Anthony and the teacher/tutor will work together to <br />motivate and coordinate services and information, prepare youth for post secondary <br />educational opportunities, provide linkages between academic and occupational learning, <br />and/or preparation for unsubsidized employment/training opportunities, as appropriate. <br />Both will meet 5 days a week, Anthony will meet 6 days a week, with WIA grant <br />recipients as a group. They will keep detail record of attitudes, behaviors and <br />performance on each youth involved in the program. The teacher will be in charge of <br />case notes for the tutoring aspect of the program. Anthony and the teacher will meet <br />once a week to compare case notes. Anthony, on weekly basis, will review each case <br />record, and develop individual case plan, accordingly. He will provide special <br />counseling for special needs (e.g., we currently have a senior high school student who <br />receives academic and career guidance counseling from Anthony). If a particular <br />problem arises that falls outside Anthony's ability to assess and intervene, he will present <br /> <br />4 <br />
