<br />'- <br /> <br />-' <br /> <br />case that a contract is awarded to the proposer, the city <br />shall have the right to duplicate, use or disclose this in- <br />formation to the extent provided in the contract. This re- <br />striction will not limit the City the right to use informa- <br />tion if it is obtained from another source. <br /> <br />XII. <br /> <br />RULES FOR PROPOSAL <br /> <br />The signer of the proposal must declare in the <br />aforementioned cover letter that the only person, persons, <br />company or parties interested in the proposal as principals, <br />are named therein; that the proposal is made without <br />collusion with any other person, persons, company or parties <br />submitting a proposal; that it is in all respects fair and <br />in good faith without collusion or fraud, and that the <br />signer of the proposal has full authority to bind the <br />proposing firm. <br /> <br />1. Proposals to be submitted shall contain the following in- <br />formation: <br /> <br />A. Credentials of the firm. Provide a synopsis of <br />the firm's qualifications, experience and <br />capacities in connection with the proposed <br />services. Clearly state what services are to be <br />performed. <br /> <br />B. List of quality control projects completed by <br />the firm. List names, locations, brief <br />description of the projects and the firm's <br />involvement, including types of testing and <br />inspection for which the firm was responsible. <br /> <br />C. Proposed staff. List the project manager and <br />the key staff members, including inspectors, <br />proposed for this project. List their responsi- <br />bilities on this project, qualifications and <br />experience. <br /> <br />D. other relevant information. <br /> <br />2. FEE SCHEDULE <br /> <br />In order to compare proposed fees on an equal basis, the <br />proposer shall complete the following FEE SCHEDULE, <br />attached as Appendix C, and submit it to the City in a <br />separate, sealed envelope. It is expressly emphasized that <br />this FEE SCHEDULE is not a bid. It is used for a cost- <br />comparison purpose only. The City reserves the right to ask <br />the selected firm to perform only part of the estimated <br />work and ask the County's testing laboratory to perform the <br />remaining work. The City does not guarantee the quantity of <br /> <br />6 <br />
