Individual Service Plan (ISP): an assessment tool that will be used to set <br />individual program and skill development goals. <br />Pre-test/Post Test is administered to students to detennine competency and <br />knowledge of the given subject matter. A comprehensive "post-test" is <br />administered to students at the end of the 8 -week program. The result <br />provides a simple and effective method of evaluating progress, knowledge <br />retention and general overall progress. <br />• Case Management: At the time of enrollment, each participant is assigned a <br />case manager who will help determine and assess progress of each student. <br />Case Managers document all performance goals and progress on a weekl <br />basis. <br />• Individual Creative Plan. monitored by the adult mentoring aspect of the <br />program. Specific artistic goals and timelines are determined and monitored in <br />conjunction with the case manager. Mentors also are actively involved during <br />this process of the student's participation in the program. <br />Project RYTMO will use all appropriate WIA MIS documents and assessment tools <br />to record and track performance standards for all primary and secondary goals for <br />each student. These will be documented by the case manager and entered into both <br />the individual case file, as well as into the VOS tracking system required by the Santa <br />Ana V7113. <br />2 How will services aid in meeting the WIA performance goals? <br />Progress toward meeting WIA performance goals are tracked through case <br />management or follow-up services when the goal attainment occurs during the exit <br />quarter. Since participant records are subject to audits, the program ensures that staff <br />receives training on how to document individual skill achievement and goal <br />attainment. <br />The services provided meet the required WIA performance goals. This is because <br />Project RYTMO is a unique and innovative approach to meeting WIA performance <br />skill attainment, diploma/credential attainment, literacy/numeric gains, and retention <br />rates. These are accomplished utilizing creative and artistic methods that engage <br />youth, while at the same time equipping youth to attain academic success and <br />occupational skills useful in the job rn, arket. <br />3 Describe how you will monitor program activity and manage for <br />performance. <br />Project RYTMO has incorporated the following methods for monitoring program <br />activity and performance: <br />
