E. Project team organization chart identifying the Project Manager and those who <br />will be performing the work along with a brief resume of each team member, <br />including similar type of projects in which he /she has been directly involved. A <br />minimum of three references for similar size and type projects, including contact <br />name, organization, and phone number, shall be included for the Project <br />Manager. The Project Manager will be the primary contact person to represent <br />the firm and will be the person to conduct the presentation, if invited for an <br />interview. Sub - consultants, if any, shall be identified with the same requirements <br />as for the prime consultant. <br />F. List of similar projects which your firm completed within the last five (5) years. <br />Project information should include project description, agency, client name along <br />with contact information, year completed, project construction and design cost, <br />and team members that worked on the projects. <br />G. Eight (8) bound copies of a complete responsive proposal shall be submitted. <br />IV. SCOPE OF WORK <br />The Consultant shall be responsible for the preparation and submittal of the <br />Environmental Reevaluation, GAD, PS &E and R/W activities through completion, <br />with the approval from the Cities of Santa Ana and Irvine and Caltrans. The <br />following is the breakdown of the scope of work for the project. Any additional tasks <br />deemed necessary by the Consultant shall be clearly identified in the proposal. <br />A. Management and Administration <br />The Consultant is fully responsible for the overall project management, <br />administration and coordination for the project, including but not limited to <br />the following: <br />Management and Coordination — The Consultant shall be the liaison <br />with affected agencies, monitor project progress and maintain project <br />files. Other responsibilities of the Consultant include supervising, <br />coordinating, monitoring and reviewing design for conformance with <br />Caltrans, County of Orange, and local agencies standards, policies <br />and procedures. <br />2. Meetings — Project Development Team (PDT) meetings shall occur <br />monthly with all stakeholders and will be held at the City of Santa Ana. <br />The Consultant shall send out the agenda and any necessary <br />communication documents a minimum of two (2) business days in <br />advance, prepare and distribute meeting minutes, new action items <br />and disposition of previous action items and other communication <br />documents within five (5) business days after the meeting to all <br />7 <br />
