Include timelines and benchmarks that will be used to track client success. Items <br />to address include the following: <br />^ State the intended outcome of proposed program <br />^ List milestones and timeline benchmarks that will be used to measure that clients are moving <br />toward stated program outcome <br />^ List the standard that will be used to measure program success (e.g., 75% of all program <br />participants will be living in permanent housing within six months of initial intake) <br />^ How will outcome measurements be taken and reported <br />^ What means will be used to take corrective action <br />Limit Response to 1,250 Words <br />CMIS benefits the community by providing region-wide data that can be used to identify gaps <br />and trends, measure success, guide strategic planning, policy, and resource distribution, and <br />meet HUD funding requirements. Homeless and at risk clients benefit because they receive <br />more comprehensive, coordinated service to help them get back on their feet. Through broad <br />based use of the system, we are able to facilitate fact based discussion of the need for basic <br />services such as shelter and food so as to inform public policy and funding decisions. Without a <br />clear strategy for obtaining accurate counts and a widely supported and adopted method for <br />capturing the information, we will continue to rely on approximations, anecdotal, incomplete and <br />often duplicated information as we make strategic decisions on funding and policy that <br />determine how we serve those most in need. <br />Expanding and supporting CMIS will allow the wide variety of basic needs agencies throughout <br />the county to track and measure a vast amount of data on their clients. Additionally, CMIS <br />supports all of the data, from each of the participating agencies, in a single centralized database <br />so that the data can be analyzed and evaluated whenever it is needed. This will allow for more <br />accurate and comprehensive measurement of countywide trends and needs. <br />The goals of OC Partnership for CMIS include: <br />^ Increase collection of factual data on homeless clients and services. <br />^ Increase number of agencies sharing data into CMIS system <br />^ Meeting requirements of HUD inputting and funding (75% of county shelters participating <br />in system) <br />We expect that the following activities will have begun within 30 days of the start of the grant <br />cycle: <br />^ Successfully bring on all required agencies <br />RFP HOMELESSN ESS PREVENTION & RAPID RE-HOUSING SERVICES 6/11/09 <br />13 <br />
