<br />Agreement #W7 -GJC-10 <br />City of Santa Ana <br /> <br />EXHIBIT A <br />TO WIA COST REIMBURSEMENT AGREEMENT <br />GREEN JOBS CORPS <br />AUGUST 1, 2009-MARCH 31, 2011 <br /> <br />GENERAL PROGRAM REQUIREMENTS <br /> <br />V. PERFORMANCE STANDARDS <br />A. Contractor shall meet or exceed required federal, State and local <br />standards, measurements and outcomes of all funding streams included in <br />this Agreement and subsequent updates. Failure to meet performance <br />levels may be grounds for funding de-obligation. <br />B. Contractor shall adhere to WIA Customer Satisfaction Policy for <br />Participants and Employers per OCWlB Information Notices No. 04- <br />OCWDA-22, OCWDA-04-16 and any subsequent updates. <br />C. Contractor shall ensure that participant activities remain uninterrupted in <br />accordance with service delivery requirements of TEGL 17-05, and any <br />subsequent updates. <br /> <br />VI. SYSTEM SECURITY AND CONFIDENTIALITY <br />Contractor agrees to the following: <br />A. That all applications and individual records related to services provided <br />under this Agreement, including eligibility for services, enrollment, and <br />referral, shall be confidential and shall not be open to examination for any <br />purpose not directly connected with the delivery of such services. <br />B. That the sharing of individual and client information necessary for <br />provision of services under this Agreement, Le.: assessment; universal <br />intake; program or training referral; job developmen"t, placement or follow- <br />up activities; and other services as needed for employment or program <br />support purposes, constitutes a valid use of such information. <br />C. That no person or employee shall otherwise publish or disclose, use, or <br />permit, cause to be published, disclosed or used, any confidential <br />information pertaining to applicants, participants, or customers overall. <br />D. That files are kept in a secure location on site and should not be removed <br />for any reason unless authorized by OCWIS Contract Administrator. <br />E. To abide by the current confidentiality provisions of respective statutes <br />and share information necessary for the administration of programs <br />operated through the OCWIB. <br />F. To abide by EDD Information Bulletin #WIAB02-78 Identity Theft. <br /> <br />VII. NON-DISCRIMINATION AND GRIEVANCE PROCEDURES <br />A. In the event individuals accessing the program file a program grievance or <br />discrimination complaint, the process for filing grievances shall be <br />determined by the services the individual received and the matter about <br />which the grievance is filed.
