<br />Our After School Youth Program includes a tutoring component that will help clients <br />improve their basic skills. Our Mentoring Program will help clients receive the guidance <br />and motivation they need to meet challenges every day. Other Youth Program's leadership <br />activities that focus on community service will help our participants grow as contributing <br />members of our community. These include activities in collaboration with Earth Resource <br />Foundation (ERF) which develops programs for Orange County youths to become involved <br />in environmental activities. <br />Our Youth Program's Career Exploration activities include guest speakers who acquaint <br />youths with the reality of employment and its prerequisites. Also, our Youth Program <br />participants are taken on tours oflocal colleges and universities throughout the course of the <br />year, thereby instilling in them a desire for higher education. <br />Our Employment Services program also has resources that will benefit Upward Mobility <br />participants, including assistance in resume writing, conducting job interviews, filling out <br />job applications, and researching job opportunities on the internet. <br /> <br />2. How the participant flow will occur. (Include the following categories:) <br /> <br />· Outreach/RecruitmentlEligibiIity Determination (Note: program operator is responsible <br />for gathering the documents required by the Service Navigator to determine eligibility) <br />The Project Manager will be responsible for recruiting program participants. <br />Recruitment will focus on several categories of potential recruits: (1) former Youth Program <br />participants; (2) relatives and friends of agency clients; (3) job seekers who come to our <br />agency; (4) clients of partner agencies. <br />Potential recruits will be contacted by phone, or in person at our agency or at their <br />homes. The recruiter will verify the eligibility of each youth contacted. The program's <br />purpose and benefits will be explained, as well as the participant's rights and <br />responsibilities. Each recruit will provide documentation of their eligibility for the program, <br />including their right to work. For those qualified recruits who agree to participate, an <br />appointment will be set for Intake/Assessment. <br /> <br />· Intake/Assessment <br />Intake documents will be prepared for each client that include contact information as <br />well as all necessary information about education and work history. Those who agree to <br />participate will sign all intake documents, including confidentiality and grievance policies <br />and procedures. Documents that establish a client's eligibility to participate and to work in <br />the U.S. will be photocopied. A client file will be created for each participant that will hold <br />all case documentation. Files will be kept in a secure location. <br />Assessment will involve interviewing each client about their occupational and career <br />goals along with their past education and work history. Each client's basic skills ability will <br />be assessed, focusing on English language ability (speaking, listening, reading, writing) and <br />basic math. <br />Based on participants' background, ability, and goals, a service plan will be developed <br />for each client that will focus on either employment or an educational program. <br />employment.
