CITY OF SANT A ANA <br />is a partner agency participating in the Coordinated Entry System's Weekly Match Meetings to e ns ure the <br />mo st vulnerable youth recei ve housing first. <br />See our Suppo1tive Service Policy in Attachment N. <br />12. Incentives <br />SUFK OC will provide incentives to youth for accomplishing program benchmarks or personal <br />goals. Incentives may include special activities ( e.g. group outings or field trips), gift cards, and <br />formal recognition such as award ce1tificates. The benchmarks we acknowledge within our <br />mentoring program include school graduation, ce1tificates earned/WEX completed, JTS s tages <br />completed, JTS/RMTS "graduation", and permanent ho us ing "move in day". We typically provide <br />incentives geared toward the following outcomes: <br />• <br />• <br />• <br />• <br />• <br />• <br />Grades in school <br />Attendance in school <br />Becoming Employed <br />Consistently attending workshops or coaching sessions for 4+ weeks <br />Developing and presenting a personal career plan or goal sheet <br />Showing leadership in a group activity or mentoring a peer ( occasionally) <br />See the SUFK OC Incentive Policy in Attachment 0. <br />13. Follow-Up Services <br />SUFK OC will provide follow-up services to youth pa1ticipants 12 months after exit. Exit is defined as any <br />patticipant who has a termination (or program "graduation") d ate within the qumter and has not received any <br />WIOA service for 90 days, except follow-up , and there are no future service s scheduled, that patticipant has <br />"exited" for the purposes of pe1formance measurement. Follow-up documen tation will be provided 1 si, 2nd, <br />3rd , and 4th quaiter after exit. <br />When a yo uth "graduates" the JTS program, they are permanently housed and self-sufficient; and they are <br />no longer receiving WIOA serv ices. The JTS mentoring programs take on average, 6 months to complete <br />after intake. Upon completion of the program , the SUFK OC mentor will typically connect w ith the youth <br />monthl y for the first 6 months and qumterly for the remainder of the follow-up year. With each connection, <br />they will en sure that the youth remains stab ly housed and self-suffic ient. <br />Our goal is for each client to ul timately find themselves in a sustainable and healthy situation. Once youth <br />are in a susta inable, healthy s ituation, we suppo11 them for potential setbacks, rel apses, new ins tances of <br />homelessness and other challenges. Therefore , we d on 't ever back out of their li ves. We determine a <br />comfo1table set of services, counseling, and other suppo1t s that a youth needs at a given point in time. All <br />youth are encouraged to remain connected to SUFK OC staff fo r the long term. Many youth in the program <br />continue their relationship as vo lunteers themselves. <br />During the follow-up period, the SUFK OC Case Manager wi ll continue to provide case management, <br />suppottive services, and incentives, as needed.
