<br /> c. Essential household items: The selected provider may assist the family with some or
<br /> all of the costs of acquiring essential household items (e.g., tableware, bedding, etc.).
<br /> d. Renter's insurance if required by the lease: The selected provider may assist the
<br /> family with some or all of the cost of renter's insurance, but only in cases where the
<br /> purchase of renter's insurance is a condition of the lease.
<br /> C. Independent Living Skills Training and Life Skills Supportive Services Required
<br /> The selected provider will be responsible for delivering the following services, including but not
<br /> limited to:
<br /> 1. Independent Living Skills Training:
<br /> a. Household Management: Training on meal planning, cooking, cleaning, laundry, and
<br /> home safety.
<br /> b. Financial Literacy: Budgeting, banking, bill payment, credit management, and savings
<br /> strategies.
<br /> c. Personal Care and Health Management: Hygiene, grooming, dressing skills, and
<br /> health maintenance.
<br /> d. Time Management and Organization
<br /> e. Transportation Skills: Guidance on using public transportation, route planning, and
<br /> obtaining driver's licenses when applicable.
<br /> 2. Life Skills Development:
<br /> a. Employment Preparation: Career exploration, resume writing, job search strategies,
<br /> interview preparation, and workplace etiquette,
<br /> b. Educational Support: Assistance with GED preparation, college applications, financial
<br /> aid navigation, and vocational training opportunities.
<br /> c. Self-Advocacy: Decision-making skills, problem solving techniques, and accessing
<br /> community resources.
<br /> d. Social Skills: Training to build healthy relationships and community connections.
<br /> e. Workshops: Organize and conduct regular workshops on various life skills topics such
<br /> as communication skills, conflict resolution, stress management, and healthy
<br /> relationships.
<br /> 3. Comprehensive Case Management Services:
<br /> a. Conduct initial assessments to identify individual needs and goals.
<br /> b. Develop personalized service plans.
<br /> c. Provide ongoing support and regular check-ins to ensure goal achievement.
<br /> d. Coordination with local agencies for additional support services as needed.
<br /> e. Work with participants to create comprehensive self-sufficiency plans.
<br /> f. Assist in setting and achieving goals related to housing, education, and career.
<br /> D. Reports
<br /> 1. Contractor shall submit a Quarterly Homeless Housing, Assistance, and Prevention Program
<br /> (HHAP) Funds Report to the Housing Authority Coordinator thirty (30) days following the end of
<br /> each quarter as listed below:
<br /> Quarter Due Dates
<br /> Quarter 1: July 1, 2025 —Se tember 30, 2025 October 31, 2025
<br /> Quarter 2: October 1, 2025 — December 31, 2025 January 31, 2026
<br /> Quarter 3: January 1, 2026— March 30, 2026 Aril 30, 2026
<br /> Quarter 4: April 1, 2026—June 30, 2026 July 31, 2026
<br /> Quarter 1: July 1, 2026—September 30, 2026 October 31, 2026
