STATEWIDE SANITARY SEWER SYSTEMSGENERAL ORDER2022-0103-DWQ <br />1.2. Spill Notification Information <br />TheEnrolleeshall provide the following spillinformation tothe California Office of <br />Emergency Services before receiving a Control Number, as applicable: <br />Name and phone number oftheperson notifying theCalifornia Office of Emergency <br />Services; <br />Estimatedspillvolume (gallons); <br />Estimatedspillratefrom the system (gallons per minute); <br />Estimateddischargerate(gallons per minute)directly into watersof the Stateor <br />indirectly into a drainage conveyance system; <br />Spillincident description: <br />o Brief narrative of the spillevent,and <br />o Spillincident location (address, city, and zip code)and closest cross streets <br />and/or landmarks; <br />Name and phone number of contact person on-scene; <br />Date and time theEnrolleewasinformedof the spillevent; <br />Name of sanitary sewer system causing the spill; <br />Spillcause or suspected cause (if known); <br />Amount ofspillcontained; <br />Name of receiving waterbodyreceiving or potentially receiving discharge; and <br />Descriptionof water bodyimpactand/ or potential impactto beneficial uses. <br />1.3. Notification of Spill Report Updates <br />Following the initial notification to theCalifornia Office of Emergency Services and until <br />such time that theEnrolleecertifies the spillreport in theonlineCIWQSSanitary Sewer <br />SystemDatabase, the Enrolleeshall provide updates to theCalifornia Office of <br />Emergency Services regarding substantial changes to: <br />Estimated spillvolume (increaseor decreasein gallons initially estimated); <br />Estimated dischargevolume dischargeddirectly into watersof the Stateor indirectly <br />into a drainage conveyancesystem(increase or decrease in gallons initially <br />estimated);and <br />Additionalimpact(s)tothereceiving water(s)andbeneficial uses. <br />ATTACHMENT E1NOTIFICATION,MONITORING,REPORTING <br />AND RECORDKEEPINGREQUIREMENTSDecember 6,2022 <br />E1 -4 <br /> <br />
