CERTIFICATIONS (ATTACHMENTS) <br />In addition to the Technical Proposal, the following forms, included in this RFP, shall be signed and <br />included as part of the proposal submittal package: <br /> Attachment A: Proposer’s Certification <br /> Attachment B: References <br /> Attachment C: Certification Regarding Debarment, Suspension, Ineligibility, and Voluntary <br />Exclusion <br /> Attachment D: Non-Collusion Affidavit <br /> Attachment E: Non-Lobbying Certification <br /> Attachment F: Non-Discrimination Certification <br /> Attachment G: SAM.gov UEI Verification <br /> Attachment H: Statement Regarding CWA Requirements <br />The proposal must be completely responsive to the RFP. Incomplete proposals will be deemed as <br />nonresponsive and will be rejected. The City reserves the right to reject any or all proposals submitted <br />and no representation is made hereby that any commitment will be awarded pursuant to this RFP or <br />otherwise. <br />PLEASE NOTE: City will not waive notarization requirement when applicable on <br />any of the required attachments. <br /> <br />IX. SELECTION PROCEDURES & CRITERIA <br />(1) The RFP Process will be open and provide sufficient time for applicants to identify an eligible <br />site and complete and submit a proposal in response to the RFP announcement. The RFP <br />Process will be open for at least one (1) calendar year with four (4) quarterly deadlines to <br />submit a proposal scheduled throughout the year. After each deadline, staff shall review any <br />Proposals to determine if the minimum Program and RFP Process requirements are met <br />(minimum threshold review). Proposals that do not meet the minimum threshold review will <br />be considered non-responsive. If the Proposal meets the minimum threshold review, staff will <br />form a Review Panel. The Review Panel for the RFP Process will consist of at least one (1) <br />employee from the City of Santa Ana Public Works Agency, Planning and Building Agency, <br />and Community Development Agency, and one (1) outside agency or government entity. If an <br />employee is not available in one (1) City Agency, a second employee may be requested from <br />EXHIBIT 1
