The Project tanager will be the primary contact <br /> person to represent the firm and will be the person to conduct the <br /> presentation, if invited for an interview. Sub-consultants, if any, shall be <br /> identified with the same requirements as for the prime consultant. <br /> <br /> F. List of similar projects which your firm completed within the last five (5}years. <br /> Project information shall include project description, agency, client name <br /> along with contact information, year completed, project construction and <br /> design costs, and team members that worked on the projects. <br /> <br /> G. Five (5) bound copies and one electronic copy in PDF format on a Compact <br /> Disc (CD} of a complete responsive proposal shall be submitted. <br /> <br /> IV. SCOPE OF WORK <br /> <br /> The Consultant shall be responsible for the preparation and submittal of the <br /> PSRE through completion, with the approval from the City and other involved <br /> agencies. The PROJECT is federally funded and Caltrans retains stewardship <br /> responsibility for project development documents to be reviewed by Federal <br /> Highway Administration (FHUVA}, the Consultant shall prepare the PSRE in <br /> accordance with the Caltrans Preparation Guidelines for Project Study Report in <br /> the Caltrans Project Development Procedures Manual (PDPM}. All necessary <br /> documentation, studies, estimates, conceptual engineering plans and exhibits <br /> shall be in accordance with the compliance documentation for California <br /> Environmental Quality Act (CEQA} and National Environmental Protection Act <br /> (NEPA}, unless otherwise noted, under the direction of a California registered <br /> Civil Engineer. The following is the breakdown of the scope of work for the <br /> PROJECT as well as the minimum deliverables required. Any additional tasks or <br /> deliverables deemed necessary by the Consultant shall be clearly identified in the <br /> proposal. <br /> <br /> A. Management and Administration <br /> <br /> The Consultant is fully responsible for the overall project management, <br /> administration and coordination of the PROJECT, including but not limited to <br /> the following: <br /> <br /> 1. Management and Coordination -The Consultant shall be the liaison with <br /> responsiblelcooperating agencies and other stakeholders, monitor project <br /> progress and maintain project files.
