Project 12-6775 <br />Request for Approval of Change Order No. 1 <br />May 20, 2013 <br />Page 3 of 4 <br />CHANGE ANALYSIS <br />DETAILED JUSTIFICATION <br />The City's Pavement Management System identified Lyon Street as a high priority for <br />rehabilitation. The pavement has deteriorated due to weather, age, heavy usage, and <br />poor drainage. Portions of the adjacent curb and gutter, driveway approaches, curb <br />ramps, and sidewalks are either missing or in disrepair and require replacement. A <br />contract was awarded to Hardy & Harper, Inc. on October 15, 2012 for the <br />rehabilitation of Lyon Street; the improvements include the reconstruction of the <br />pavement, replacement of damaged concrete curb and gutter, driveway approaches, <br />and sidewalk, and the construction of new wheelchair ramps. <br />The original contract did not include this location but prices were agreed upon per the <br />cost analysis below. Although the request was for a concrete bus pad, there was <br />incidental work that needed to be completed. The driveway approach at 1131 South <br />Grand Avenue and adjacent to the bus pad could not be salvaged due to cracking and <br />had to be replaced.
