July 1, 2013 - <br />June 30, 2015 salary costs are based on total hours worked that includes <br />anticipated furlough hours and days the city offices are closed for <br />Workforce Specialist II and Fiscal Specialist positions. <br />(2) Employee Benefits include health insurance, dental insurance, life <br />insurance, and long-term disability insurance. Also included are payroll <br />taxes such as FICA, Federal Unemployment Tax, State Unemployment Tax, and <br />Worker's Compensation Tax, based on the currently prevailing rates. <br />(3) Enrollments in WEX activities are contingent upon availability of funds <br />and shall consist of a minimum of fifty-nine (59) enrollments for the <br />period of July 1, 2013 through June 30, 2014 and fifty-nine (59) <br />enrollments for the period of July 1, 2014 through June 30, 2015. <br />(4) Benefits costs included are payroll taxes such as FICA, Federal <br />Unemployment Tax, State Unemployment Tax, and Worker's Compensation Tax, <br />based on the currently prevailing rates. <br />(5) Enrollments in VTR activities are contingent upon availability of funds <br />and shall consist of a minimum of fifty (50) enrollments for the period <br />of July 1, 2013 through June 30, 2014 and fifty (50) enrollments for the <br />period of July 1, 2014 through June 30, 2015. <br />14.2 CONTRACTOR and ADMINISTRATOR may agree, subject to advance written <br />notice to add, delete, or otherwise modify line items and/or amounts and/or <br />the number and type of FTE positions without changing COUNTY'S maximum <br />obligation as stated in Subparagraph 20.1 of this Agreement or reducing the <br />level of service to be provided by CONTRACTOR. Further, in accordance with <br />Subparagraph 44.3 of this Agreement, in the event ADMINISTRATOR reduces the <br />maximum obligation as stated in Subparagraph 20.1, CONTRACTOR and <br />ADMINISTRATOR may mutually agree in writing to proportionately reduce the <br />service goals as set forth in this Exhibit A to this Agreement. <br />15. <br />STAFF <br />CONTRACTOR shall employ staff with experience in placing participants <br />(WAM0313) <br />14 of 22 <br />(April S, 2013)
