The Contractor shall include street lighting maintenance personnel's qualifications in the <br />proposal. During the term of the contract the City will notify the Contractor, in the City's <br />sole opinion, if personnel proposed for any maintenance are not qualified to work on <br />these systems. <br />3. List of Street Lighting Systems <br />For the purpose of this maintenance contract, the City of Santa Ana street light Inventory <br />is as shown on the maps in Appendix B and all safety lights in conjunction with the traffic <br />signal locations listed in Appendix A. <br />4. Modifications to System <br />The City reserves the right to add or delete sections or circuits of streetlights as they are <br />added or deleted from the City system. <br />5. Extraordinary Maintenance <br />(a) The failure or malfunction of street lights may be considered extraordinary <br />maintenance. <br />(b) Any upgrading, repair work, or any installation solicited by the City, shall be <br />considered as extraordinary. Any extraordinary maintenance requests from the City <br />shall be confirmed by the Contractor as to the time of and number of personnel <br />dispatched. The Contractor shall contact the Streetlight Management Section <br />regarding any extraordinary maintenance work found to be beyond requested repairs <br />and seek prior approval before the work is scheduled. Contractor shall notify <br />Public Works Agency (PWA) Dispatch Office at 714.647.3380 each day prior to <br />crews being dispatched by Contractor <br />(1) No underground work shall be performed prior to completion of Underground <br />Service Alert (USA) markings. <br />(ti) An electrical permit may need to be obtained prior to any additions to the City's <br />street light or other electrical system. <br />(iii) All extraordinary work completed by Contractor shall be invoiced time and <br />material as specified on Contractor's bid proposal <br />(c) Contractor shall respond to requests by City for extraordinary work within 24 hours <br />of the request. Work shall start in a timely manner. Contractor shall dispatch <br />personnel, materials and equipment on a daily basis until work is completed unless <br />conditions do not allow work to be performed. <br />(d) City shall pay for such extra ordinary repairs required as follows: <br />(i) Materials. The City shall pay to the Contractor for any materials used in <br />extraordinary maintenance at Contractor's cost from the supplier plus the <br />percentage specified In the Contractor's bid proposal. All materials and <br />parts shall be new or have the approval of the Streetlight Management Section if <br />otherwise. The City has the right to inspect the Contractor's records to verify any <br />material costs used in extraordinary maintenance. The City may provide <br />replacement poles and fixtures. <br />(ii) Direct Labor: Contractor shall furnish a monthly report of hours spent in <br />extraordinary maintenance of street light and appurtenances per address. City <br />shall pay the Contractor for such hours of extraordinary maintenance Performed <br />at the iob site address at the rates specified on the Contractor's bid proposal. <br />Crews shall be dispatched so as to maximize work performed. Said hourly rates <br />shall be the total cost to the City. Rates shall Include all compensation for <br />wages, profit, overhead, any fringe benefits such as employer payments to, or on <br />
