WIOA YOUTH RFP QUESTIONS <br />Instructions: Please provide a narrative response for each section and maintain the formatted sequence <br />according to the headers. Please answer each question as concisely as possible. Use font size 12 and <br />standard size margins. 20 pages maximum <br />Experience /Qualifications (1 page) <br />A. Briefly describe your organization. <br />B. List the facility address and address of headquarters if different location. Please <br />include major cross streets. <br />C. Indicate hours of operation for facility. <br />D. Briefly indicate what makes your facility youth friendly and suitable for the <br />proposed program. <br />E. Provide brief information on the following elements: <br />1. Average annual operating budget and sources of revenue <br />2. Brief description of staff <br />3. Past experience with WIOA programs or other youth programs <br />4. What kind of impact has your organization has made on the youth <br />community <br />II. Proposed Program <br />A. Provide an overview of the program's main objectives. <br />B. Please provide a full description of the program in sections III -XV. <br />NOTE: It will be expected that a full -time case manager operate this program. <br />III. Population <br />A. Describe the total number of youth that will be served by this grant. <br />B. Describe the age range of the youth that will be served. <br />C. Describe any minimum requirements that youth need to have in order to be <br />enrolled into your program (aside from the WIOA Youth eligibility requirements). <br />D. Youth Providers are required to enroll 75% of out -of- school youth. <br />E. Special consideration for organizations that can serve the following youth <br />populations: <br />1. At least 75% of Out -of- School youth (required) <br />2. At least 10% of youth with disabilities <br />3. At least 10% of foster /emancipated youth <br />4. At least 10% youth that have dropped out of high school <br />5. At least 10% of youth on probation <br />F. Specify how the targeted youth populations (out -of- school, youth with disabilities, <br />foster /emancipated, probation, dropouts) will be recruited. <br />G. Include a Letter(s) of Support from organizations that have agreed to refer youth <br />to your program as <br />H. Describe your recruitment plan that will fulfill the requirement of enrolling 100% <br />of youth by the 2nd quarter of the program (December 31, 2015). <br />19D -22 18 <br />
