RESOLUTION NO. 79-]77 <br />PAGE TWO <br /> <br />Ana Deferred Compensation Plan, and subject further <br />to the conditions that: <br /> <br /> i. Said officer shall not have any <br />vested right in the amounts contributed <br />by the city until termination of employ- <br />ment with city; <br /> <br /> ii. City will pay the total amount <br />of contributions plus any earnings thereon <br />to said officer upon termination, resigna- <br />tion, retirement, dismissal or death. <br /> <br />(b) Health Insurance <br /> <br /> The city will contribute the following <br />amounts toward the payment of the premiums for <br />said officer and dependents, if applicable, under <br />the existing health insurance program (or a new <br />program providing substantially similar coverage <br />and benefits): <br /> <br /> i. If the officer aforesaid does <br />not have dependents included under the <br />program, one hundred percent (100%) of <br />the premiums; <br /> <br /> ii. If the officer aforesaid has <br />dependents included under the program, <br />an amount equal to ninety percent (90%) <br />of the total premium applicable to the <br />"family plan" covering said officer and <br />his or her dependents. <br /> <br />(c) Disability Insurance <br /> <br /> The city shall pay the full premium for a <br />long-term disability insurance plan for said <br />officer. <br /> <br />(d) Dental Insurance <br /> <br /> The city shall contribute up to a maximum <br />of Fifteen Dollars Thirty-two Cents ($15.32) <br />per month toward the payment of premiums for <br />dental insurance plans provided by the city for <br />said officer and any of his or her eligible de- <br />pendents. <br /> <br />(e) Life Insurance <br /> <br /> Effective September 1, 1979, or as soon <br />thereafter as practicable, the city shall seek <br />to amend the group term life insurance policy <br />it maintains on behalf of its employees in <br />order to provide said officer with term life <br />insurance coverage in an amount equal to twice <br />said officer's annual rate of salary, provided <br /> <br /> <br />
