RESOLUTION NO. 79-]77 PAGE TWO Ana Deferred Compensation Plan, and subject further to the conditions that: i. Said officer shall not have any vested right in the amounts contributed by the city until termination of employ- ment with city; ii. City will pay the total amount of contributions plus any earnings thereon to said officer upon termination, resigna- tion, retirement, dismissal or death. (b) Health Insurance The city will contribute the following amounts toward the payment of the premiums for said officer and dependents, if applicable, under the existing health insurance program (or a new program providing substantially similar coverage and benefits): i. If the officer aforesaid does not have dependents included under the program, one hundred percent (100%) of the premiums; ii. If the officer aforesaid has dependents included under the program, an amount equal to ninety percent (90%) of the total premium applicable to the "family plan" covering said officer and his or her dependents. (c) Disability Insurance The city shall pay the full premium for a long-term disability insurance plan for said officer. (d) Dental Insurance The city shall contribute up to a maximum of Fifteen Dollars Thirty-two Cents ($15.32) per month toward the payment of premiums for dental insurance plans provided by the city for said officer and any of his or her eligible de- pendents. (e) Life Insurance Effective September 1, 1979, or as soon thereafter as practicable, the city shall seek to amend the group term life insurance policy it maintains on behalf of its employees in order to provide said officer with term life insurance coverage in an amount equal to twice said officer's annual rate of salary, provided
