RESOLUTION NO. 80- 135 <br />PAGE TWO <br /> <br />program, one hundred percent (100%) of <br />the premiums; <br /> <br /> ii. If the officer aforesaid has <br />dependents included under the program, <br />an amount equal to ninety percent (90%) <br />of the total premium applicable to the <br />"family plan" covering said officer and <br />his or her dependents. <br /> <br />(c) Disability Insurance <br /> <br /> The city shall pay the full premium for <br />long-term disability insurance plan for said <br />officer. <br /> <br />(d) Dental Insurance <br /> <br /> The city shall contribute up to a maximum <br />of Fifteen Dollars Thirty-two Cents ($15.32) <br />per month toward the payment of premiums for <br />dental insurance plans provided by the city for <br />said officer and any of his or her eligible de- <br />pendents. <br /> <br />(e) Life Insurance <br /> <br /> The city shall provide said officer with term <br />life insurance coverage in an amount equal <br />to twice said officer's annual rate of salary, <br />provided said officer can provide evidence of <br />insurability if so required. Said life insurance <br />coverage shall include an Accidental Death and <br />Dismemberment (AD and D) provision. The city <br />shall pay any reasonable additional premium <br />cost to provide such life insurance coverage <br />at no cost to said officer. <br /> <br /> In the event that said officer is deter- <br />mined to be ineligible for life insurance cover- <br />age in an amount equal to twice the annual rate <br />of salary, the city shall attempt to provide as <br />much additional coverage above existing amounts <br />as may be obtained without said officer having <br />to provide evidence of insurability, but in no <br />case shall the amount of coverage be less than <br />the existing Twenty Thousand Dollars ($20,000.00) <br />and AD and D coverage. <br /> <br />(f) Carry Over Benefits. <br /> <br /> In addition to the benefits specified in <br />subparagraphs (a) through (e) hereinabove, said <br />officer shall receive any and all employee bene- <br />fits accorded unrepresented executive management <br />employees of the city who are miscellaneous mem- <br />bers of the Public Employees' Retirement System. <br /> <br /> <br />
