The City shall provide as <br />additional compensation in the event <br />temporary housing is required by said <br />officer, all expenses for such temporary <br />housing which exceed the sum of One <br />Thousand Dollars ($1,000.00) per month, <br />provided, however, in no event shall <br />such sum exceed Five Hundred Dollars <br />($500.00) per month. <br /> <br />(f) Moving Expense <br /> <br /> The City shall pay for all reasonable <br />and necessary moving expenses incurred by <br />said officer in relocating from the State of <br />Maine to the City of Santa Ana. <br /> <br />(g) Dental Insurance <br /> <br /> The City will contribute up to a maxi- <br />mum of Fifteen Dollars Thirty-two Cents <br />($15.32) per month toward the payment of <br />premiums for dental insurance plans provided <br />by the City for said officer and any eligible <br />dependents. <br /> <br />(h) Life Insurance <br /> <br /> The City will seek to amend the group <br />term life insurance policy it maintains on <br />behalf of its employees in order to provide <br />said officer with term life insurance <br />coverage in an amount equal to twice said <br />officer's annual rate of salary, provided <br />said officer can provide evidence of <br />insurability if so required. Said life <br />insurance coverage shall include an <br />Accidental Death and Dismemberment (AD & D) <br />provision. The City will pay any reasonable <br />additional premium cost to provide such life <br />insurance coverage at no cost to said <br />officer. <br /> <br /> In the event said officer is determined <br />to be ineligible for life insurance coverage <br />in an amount equal to twice the annual rate <br />of salary, the City will attempt to provide <br />as much additional coverage above existing <br />amounts as may be obtained without said <br />officer having to provide evidence of <br />insurability, but in no case shall the amount <br />of coverage be less than the existing <br />$20,000.00 AD & D coverage. <br /> <br />(i) Carr~ Over Benefits <br /> <br /> In addition to the benefits specified in <br />subparagraphs (a) through (h) hereinabove, <br />said officer shall receive any and all <br />benefits accorded unrepresented executive <br />management employees of the City who are <br />miscellaneous members of the Public <br />Employees' Retirement System (PERS). <br /> <br /> <br />
