PART I: ANNUAL OBJECTIVES <br /> <br />A. Guidelines for Developing Objectives <br /> <br />At lease one month prior to your anniversary date~. <br />you and your supervisor must develop your one-year <br />objectives (at least 5). <br /> <br />List objectives in order of importance. Each <br />objective must represent a specific outcome and <br />state when the objective will be completed. <br /> <br />Assign a relative weight to each objective. Must <br />be at least 10%, and all must total 100%. <br /> <br />4. Identify how the.objective will be measured. <br /> <br />Guidelines for Evaluating Employee at 6 month (and <br />quarterly) review <br /> <br />1. Review objective and determine level of achievement. <br /> <br />2. Identify specific progress to date, areas of <br /> improvement needed or modification of objective. <br /> <br />Both supervisor and employee to note, sign and <br />date comments, modified objectives and standards, <br />and plan for improvement. <br /> <br />C. Guidelines for Rating Employee at end of year <br /> <br />1.. Circle performance rating (see definitions) for <br /> each objective. <br /> <br />Multiply the rating .by the assigned relative <br />weight.. <br /> <br />3.
