Project 04-6375
Request for Approval of Change Order No. 1
March 30, 2007
Page 3
thermoplastic polyolefin membrane roof system. Prior approval was granted
on November 2, 2006. (PCO No. 7)
AGREED PRICE = $20,177
Item 8. While doing the selective demolition Parks, Recreation and Community
Services Agency had requested to modify the exterior columns and walls to
accommodate the building use. Prior approval was granted on November
14, 2006. (PCO No. 9)
AGREED PRICE = $2,534
Item 9. The contractor was advised by SCE that the point of connection for the
power was incorrect and was only available from the northeast corner of the
building not the southwest west corner, as shown on the plans. To mitigate
the cost to new conduit and wire from the opposite side of the facility, Parks,
Recreation and Community Services Agency agreed to relocate the
switchgear to the courtyard. Prior approval was granted on November 14,
2006. (PCO No. 10)
AGREED PRICE = $18,883
Item 10. Parks, Recreation and Community Services Agency had requested to have
conduit installed for its future security system. Installing the conduit during
the construction will allow the conduit to be concealed within the walls and
the roof. Prior approval was granted on November 14, 2006. (PCO No. 11)
AGREED PRICE = $18,832
Item 11.The new door to room 121 would not fit within the existing opening as shown
on the contract drawings. Parks, Recreation and Community Services
Agency had requested to modify the wall to accommodate the new door and
replace the glass in that room and the adjacent room 120. Additionally, the
new plumbing in the restroom will not fit within the existing walls; therefore,
requiring the walls to be furred. Prior approval was granted on November 14,
2006. (PCO No. 13)
AGREED PRICE = $7,142
Item 12.The contract document inaccurately dimensioned the restrooms and the
utility room; consequently, the drinking fountains and the water heater would
not fit within the designed spaces. Parks, Recreation and Community
Services Agency had requested to eliminate the drinking fountains and
replace the existing water heater with a tankless model. Additionally, Parks,
Recreation and Community Services Agency had requested to extend the
new gas line to accommodate future construction. Prior approval was
granted on November 14, 2006. (PCO No. 15)
AGREED PRICE = $ -0-
