RESOLUTION NO. 79-163 <br />PAGE TWO <br /> <br /> (c) Dental Insurance. The City will continue to <br />contribute up to a maximum of fifteen dollars and thirty-two <br />cents ($15.32) per month toward the payment of premiums for <br />dental insurance plans provided by the City for said officer <br />and any eligible dependents. <br /> <br /> (d) Dis~__bilit_~. Insurance. The City will continue <br />to pay the full premium for the existing long-term disability <br />plan provided by the City for said officer. <br /> <br /> (e) Life Insurance. Effective September 1, 1979, <br />or as soon thereafter aa practicable, the City will seek to <br />amend the group term life insurance policy it maintains on <br />behalf of its employees in order to provide said officer <br />with term life insurance coverage in an amount equal to <br />twice said officer's annual rate of salary, provided said <br />officer can provide evidence of insurability if so required. <br />Said life insurance coverage shall include an Accidental <br />Death and Dismemberment (AD&D) provision. The City will pay <br />any reasonable additional premium cost to provide such life <br />insurance coverage at no cost to said officer. <br /> <br /> In the event that said officer is determined to be <br />ineligible for life insurance coverage in an amount equal to <br />twice the annual rate of salary the City will attempt to <br />provide as much additional coverage above existing amounts <br />as may be obtained without said officer having to provide <br />evidence of insurability, but in no case shall the amount of <br />coverage be less than the existing $20,000 AD&D coverage. <br /> <br /> (f) ~_~ver Benefits. In addition to the bene- <br />fits specified in subparagraphs (a) through (e) hereinabove, <br />said officer shall receive any and all benefits accorded un- <br />represented executive management employees of the City who <br />are miscellaneous members of the Public Employee's Retirement <br />System (P.E.R.S.). <br /> <br />5.
